Open Enrollment Period 2025
1. What is the open subscription period?
This is the period that ASES establishes annually for Vital Plan policyholders to make the changes they deem pertinent to attend to their medical needs without the need for just cause. Specifically, changes of insurer, Primary Medical Groups and Primary Care Physicians. This period is established in compliance with the 42 CFR of CMS, which is the regulatory entity.
2. When is the open enrollment period?
The open enrollment period begins on January 1, insurer but and runs through February 14, 2025.
3. What changes can I make?
You can change your Vital insurer, your primary medical groups and primary care physician.
4. How do I make these changes?
If you are changing carrier insurer, you must contact the ASES Enrollment Counselor at 1-833-253-7721. If you are staying with the same insurer but are changing your primary medical group or primary care physician, you can contact your insurer at the 800 Customer Service line on the back of your card.
5. After February 14, 2025, can I make changes?
After the open enrollment period, changes can be made as long as there is just cause. For example, difficulty in accessing covered services and problems with the provider network, among others. These cases require prior authorization from ASES. To contact the ASES Customer Service call center, you must dial 787-474-3300 option 1. Our Service Representatives will be ready to assist you.
