This section of the portal gives you the opportunity to do a search for providers for your records. In the end, you can print them to have with you when you need them. Your selections will disappear when you leave this page. If you want to make your unique Insurer change today, start your provider search by clicking on the “Provider Search" box.

Selection of my Primary Physician
You need to select a primary care physician (PCP) or Primary Medical Group (PMG) before you can change Insurers. Use the Provider Search link to find your PCP, PMG, specialist, hospital or other health care facility near you. Before selecting a new Insurer, make sure your PCP, PMG, hospital or other health care service providers are in the provider's network of the Insurer of your choice.

Provider Search

A primary care physician (PCP) is a doctor who is selected by you and who will assist you in your health care and refer you to a specialist when necessary.

Primary Medical Group (PMG) is a group of doctors who help organize your health care services and work with your Insurer to make sure you get the care you need. Your ID card shows the name of your PCP and the number of your Primary Medical Group.

When you enroll with an Insurer, you will need a primary care physician (PCP) to coordinate your medical care. Your PCP plays a very important role in your medical care and the health services you will receive. For this reason, you must choose a PCP by selecting any of the participating Insurers of the VITAL Health Plan.

It is important that you choose the PCP that is best for you. Remember that you need to select a primary physician (PCP) before you can make an insurance change. Here are some considerations to help you select a PCP:

Choose your primary doctor from the following options:

Adult beneficiaries Choose among the following types of practitioners as your PCP: general practice, family practice, internal medicine, or geriatric medicine.
Beneficiaries under age 21 Consider choosing a pediatrician as your PCP.
Females older than 12 years old Female Enrollees age twelve (12) and older have an option to directly choose an obstetrician/gynecologist (OB/GYN) as a PCP with their selected MCO.

To change your Insurer, click on the "Log in now" link.

If you want to use a new primary care physician (PCP) who is already in your assigned Insurer, you do not need to change Insurers.

If you want to go to a PCP or specialist who is not part of your Insurer, you will need to change Insurers. Remember, you can only make one change during the annual open enrollment period, so chose carefully. When changing Insurers, you will need to select a PCP. Also remember everyone in your family doesn't have to use the same Insurer.

You must find a PCP and a Specialist who are in the same Insurer. Remember that all providers must be contracted by your Insurer, that is, belong to your provider network.
