It does not provide details on eligibility requirements under the Medicaid program, the VITAL Health Plan, benefits, exceptions, general terms, or limitations of the Puerto Rico Government Health Plan. Eligibility determinations for the VITAL Health Plan are made by the Puerto Rico Medicaid program. This portal does not represent a contract between you and the Puerto Rico Health Insurance Administration (ASES), the Puerto Rico Medicaid program, or any participating insurers (MCOs).

The information about provider networks presented on this portal is provided to ASES by insurers contracted for the VITAL Health Plan. This portal does not modify the rules, benefits, exceptions, general terms, or limitations of the Medicaid Program or the Puerto Rico Government Health Plan. Periodic updates to the provided information will be made within 48 hours.
